What is MY LIFE KIT?

MY LIFE KIT is an all-in-one booklet that was created to help others help you. It contains pages for your personal information and medical history as well as advance care planning documents, There is also a list of emergency and essential phone numbers.

Why did we make MY LIFE KIT?

In the case of emergency, medical responders often need to know some basic personal and medical information about you. However, during emergencies there may be instances where you cannot answer questions yourself. MY LIFE KIT is designed to hold all the personal and medical information that a responder, family member, or friend would need to best help you in an emergency.

Who is MY LIFE KIT for?

While MY LIFE KIT is ideal for seniors, people with chronic illnesses, and people who live alone, we encourage everyone to fill out the Kit. Medical emergencies can happen to people of all ages and abilities, and MY LIFE KIT is an easy way to prepare

How much does MY LIFE KIT cost?

MY LIFE KIT is provided free of charge through the generous contributions of the BC Association of Community Response Networks (BCCRN). The BCCRN is a provincial network working to stop elder abuse and neglect across British Columbia. Marpole Oakridge Family Place hosts a chapter of the BC Community Response Network.

Where to keep MY LIFE KIT?

Place MY LIFE KIT on the fridge with the strong magnet provided. Inside MY LIFE KIT, there is also a sticker decal. This can be placed just inside the front door of your house, so that emergency responders know that you have a completed MY LIFE KIT on the fridge in the kitchen.

Why is there a travel section for MY LIFE KIT?

We have recently updated MY LIFE KIT to include a travel checklist, travel health card, and personal health card. Medical emergencies can also happen abroad, and if you are in a different country and experiencing an emergency, there are additional factors to consider such as health insurance and payment, potential language barriers, a lack of local support, and more.

The MY LIFE KIT travel checklist aims to help you prepare for your trip abroad by reminding you of several important pre-departure steps that should be taken to ensure your safety while travelling. These steps include checking your destinations’ entry and exit requirements, purchasing travel insurance, getting necessary vaccinations, and designating an emergency contact.

The MY LIFE KIT travel health card is meant to be filled out and kept on your person while travelling abroad. On this card, you can fill out your basic medical and travel insurance policy number and contact information. This is important to have on you when you travel, as you might need it to access medical treatment abroad. Furthermore, in many cases you must alert your insurance provider of a medical emergency within 24 hours. Requirements vary between different insurance policies, so please read through your policy carefully before your trip.

The MY LIFE KIT personal health card can be kept on your person during your everyday life, both abroad and at home. It includes space to fill out your medical conditions and emergency contacts. This information allows responders to alert your designated emergency contact and help you quickly in the instance of an emergency that happens outside of the home.

I already filled out MY LIFE KIT. Do I need a new one?

It depends. MY LIFE KIT is most effective when it has up-to-date personal information, medical history, and desired emergency response procedures. If any of the information you have listed has recently changed (for example, new medications or a new address), we encourage you to update your MY LIFE KIT as well. Regardless, we recommend reviewing your MY LIFE KIT annually to ensure that the information you have written is still accurate.
